    General Summary:


    The Admissions and Discharge Nurse is responsible for administrative tasks, such as collecting medical information and ensuring that all papework is in order for admission, discharge, and/or transfer.

    The Admissions and Discharge Nurse will also provide patient care as needed.

    This individual will work with a team of other medical professionals and may administer medicine, start IV's and other complete other advanced skills when necessary.

    The Admissions and Discharge Nurse will also educate patients on various medical conditions and/or procedures during their hospital stay.


    Essential Functions:

    • Assess patients and complete all admission forms per hospital policy.
    • Assess and diganose patient and fmaily needs to provide quality care to patient(s).
    • Develops, discusses, and communicates a realistic plan of care for each patient, in collaboration with each patient/support person to address all identified needs.
    • Obtain medical history from patients upon admission and completes medical history documentation in the EMR.
    • Educates patients on operations, treatment plan and diagnosis.
    • Demonstrates the skills and judgement necessary to implement medical plan of care, nursing interventions, and procedures as necessary for the care of the patient.
    • Evaluates the identified problems, care provided and patient's responses are reassessed in order to meet patient and family needs to ensure overall quality of care delivered.
    • Maintains up to date and accurate documentation of nursing care provided to ensure the integration of information for use by the healthcare team, to ensure quality care.
    • Maintains current knowledge/certification/licensure. Pursues professional growth and development.
    • Directs other licensed and non-licensed personnel in the provision of care to patients.
    • Demonstrates respect and regard for the dignity of all patients, families, and fellow employees to ensure a professional, responsible, and courteous environment.
    • Attends all required safety training programs and can describe his/her responsibilities related to general safety, department/service safety, and specific job related hazards.
    • Performs all responsibilities/duties required by the unit as specific to the scope of service to ensure that the unique nature of the client is addressed. This includes, but not limited to, teh age of the client served.
    • Follows the hospital exposure control plans/bloodborne and airborne pathogens.
    • Promotes effective working relations as part of a department/unit team interdepartmentally to facilitate the departments/units ability to meet its goals and objectives.
    • Functions as a role model and provides support and development for current and new staff.
