- Must be skilled in pain and symptom management.
- Able to triage patient needs and make patient visits as appropriate.
- Provide skilled and supportive nursing care to patients and families.
- Perform assessment of patient and family, through observation, interviews, and analysis of records
- Respond to physical, emotional psychosocial, environmental, needs.
- Identify patient and family needs for other home health care services and makes referrals through agency-designated channels
- Identify signs and symptoms of the disease and explains diagnosis and the nature of palliative care to the patient and family as appropriate
- Utilize appropriate nursing interventions to alleviate pain and discomfort in accordance with the plan of care.
- Supports decision making by the patient and primary caregivers
- Minimum of two years of nursing experience, preferably in medical/surgical nursing required Valid Clean Driver's License Required Home Health Care and/or Hospice / End of life care experience preferred Active client contact within past three years, preferred. Certified Hospice and Palliative Care Nurse (CHPN) preferred
