Oak Street Health is building a network of neighborhood primary care centers to provide equitable and effective value-based healthcare to local Medicare patient populations. Our Primary Care Physicians focus on delivering high-quality care rather than volume services, working within our value-based care model that rewards outcomes and drives low hospital admissions.

Responsibilities

Assess and diagnose patients at local clinics
Oversee direct administer primary care prescribe pharmaceutical treatments maintain patient electronic medical record data collaborate with regional leadership meet health quality goals work practice managers direct center team other duties assigned

Medical Doctor M.D. or Doctor Osteopathy D.O. Graduate Internal Medicine Family Medicine Board Certification Or board eligible Active non-probationary unrestricted State License Active DEA license US work authorization
