    GENERAL RESPONSIBILITIES:
    Works as a multidisciplinary team member under the direct supervision of the Program Director or designee. Completes screenings, evaluations and assessments of high quality. Develops individualized treatment plans with patients, facilitates psycho-educational groups and other groups as assigned. Consults with other service providers with respect and in the best interest of the patients. Actively participates in improving the quality of care whenever possible. Recognizes and adheres to the CASAC Canon of Ethical Principles as necessary.


    SPECIFIC RESPONSIBILITIES:


    Specific tasks assigned are aligned with professional standards and skills set by OASAS as knowledge, skills and professional expectations for a NYS CASAC.Maintain the ability to utilize the latest edition of the Diagnostic and Statistical Manual of Mental Disorders in the assessment of individuals with substance abuse disorders.

    Maintain knowledge and skill in the application of the (12) twelve core functions of the substance abuse counselor. Broad understanding of Methadone Treatment and Co-existing Disorders.
    Maintain a caseload of patients, as assigned.
    Evaluates and motivates the patient.

    Assists patients in developing an individual treatment plan in conjunction with the multidisciplinary treatment team and other community service provider as appropriate.

    Provides individual and group counseling.
    Maintains and protects accurate and timely documentation in case records according to agency policy.
    Assists in providing a safe and respectful environment for treatment.
    Attends and participates in all assigned meetings and committees.
    Assists in providing professional assistance with and to interns as Qualified Health Professional and consultant.

    Develops and networks with other community service providers to provide the full continuum of care needs identified by the multidisciplinary treatment team and supervisor.

    Makes and completes all referrals to other community service providers as appropriate.
    Continues to seek and develop professional growth and activities.
    Take monitored urine sample of clients.
    Maintains a professional relationship with patients, peers, other providers and the community.
    Encourages and provides introduction to self -help programs and any other community resources appropriate to patients' needs.
    Maintains personal health and position as a positive role model.
    Demonstrates excellence in both internal and external customer service.
    Understands and is able to effectively communicate HIPAA compliance, corporate compliance and client confidentiality.
    Ensures and/or remains in compliance with local, state, and federal regulation, i.e. DHHS HRSA and NYSDOH, and all accreditation standards (e.g. Joint Commission and NCQA-PCMH).Adheres to the National Patient Safety Goals as defined by the Joint Commission and Whitney M. Young Jr. Health Center.
    Completes other duties as assigned.


    RequirementsMINIMUM QUALIFICATIONS:
    NYS CASAC-T in good standing/current certifications. Ability to maintain professional boundaries. Ability to maintain strict confidentiality with good grasp of ethical codes/conduct. Ability to work with frequents interruptions. Ability to withstand environmental stress. Must have good listening, and communication skills. Must have good written and verbal skills.


    PREFERRED QUALIFICATIONS:

    Salary range:
    $ $20.50 hourly
