Description
Are you an experienced primary care nurse practitioner (NP) or physician assistant (PA) who aims to make a difference in the lives of your patients by providing quality care? TeamHealth is hiring a full-time nurse practitioner (NP) or physician assistant (PA) to work in a skilled nursing facility environment in the Seattle area rounding on patients 5 days per week.
Perform primary care in a skilled nursing facility setting for short term and long-term patientsManage chronic disease, diagnose illnesses and medical conditions, and implement plan of care
Consult with physicians and coordinate patient care with other healthcare providers
Ability to collaborate with the healthcare team
1 or more years of experience in primary care
~ Post-acute care experience a plus but not required
~ This full-time role is benefit eligible.
