- Uses systematic approach to provide nursing care to patients and families/populations by performing both focused and comprehensive assessments through subjective and evidence-based objective data.
- Prioritizes, synthesizes, and documents assessments and nursing plan of care using critical thinking and judgment.
- Exercises judgment while coordinating care and referring patients to agencies or health care professional for other services/transitions in care where indicated.
- Performs telephone triage using best practice guidelines and documentation to ensure patients receive the right level of care in the right setting at the right time by the right caregiver using the right equipment and supplies.
- Uses evidence-based protocols to provide appropriate patient advice and education.
- Responds to patient needs according to medical priority.
- Assists the patient by acquiring the appropriate appointment, as needed.
- Communicates results to patients, documenting accurately and completely.
- Provides next day follow up on sick patients for progress update, documenting feedback.
- Manages all medication refill requests or forwards to provider as warranted.
- Reviews lab and radiology results when provider is out of the office, forwarding abnormal findings to on-call provider.
- Follows organizational standing orders.
- Manages patient portal questions.
- Confers with provider regarding any incomplete patient tests/consults or other incomplete orders prior to patient visit.
- Coordinates care with disease management or case programs as needed.
- Assists with scheduling tests and treatments, and processing referrals to other providers.
- Participates in professional development activities and maintains professional affiliations.
- Creates and maintains patient records and other information as needed.
- Interacts with patients, families, staff, and providers in a professional, positive, and respectful manner.

Description
Under direction of the Clinical Director of RHCs, the Triage Nurse is part of the collaborative, interdisciplinary team, providing patient-centered, evidence-based care, which includes assessment, nursing diagnosis, outcome identification, planning, implementation, and evaluation. The Triage Nurse answers and addresses incoming patient calls, communicates results, addresses medication refill requests, and manages patient portal questions.Essential Duties and Responsibilities:
Education: Graduate of an accredited Registered Nursing program.
Licensure/Certification: Current Licensure through State of Indiana required. CPR certification required.
Experience: One to three years of clinical experience required. Knowledge of medical equipment and instruments, common safety hazards and precautions and treatment room procedures required. Ability to maintain strict confidentiality.
