    Foster Care Case Manager

    Full-time hours a week. Typically M-F & On-call rotation

    Hiring range:
    $ $21.97. depending upon experience

    Minimum requirements for this position


    • Bachelor's degree in social or behavioral sciences.

    OR


    • Bachelor's degree in a non-social or behavior science, and,
    • Two years experience supervised by an individual with a MSW or other master's degree in the social or behavioral sciences.

    Position Summary

    The Case Manager is responsible for the case coordination of foster clients and foster parents.

    Essential Duties and Responsibilities under guidance of TFC supervisor and manager


    • Provide case management for individual children, coordinate services for child(ren) and their family and provide monitoring and support to foster homes.
    • The case manager, under the direct supervision of the placement supervisor, is responsible for the monitoring and protection of children.
    • Establish, build, and maintain rapport with children in care, foster homes, kin, and other members of the multidisciplinary team in order that optimal information gathering, and collective decision making may occur. These duties must be carried out in a manner that respects the rights and dignity of all involved.
    • Monitor and ensure safety of clients and assist/advocate for them to reach their goals and full potential.
    • Coordinate case with providers and support goals of Family Service Plan.
    • Maintain a clinical record for each client/family per agency standards and adhere to volume 7 regulations ensuring files are always audit ready.
    • Responsible for case management, including the coordination of services, case related treatment team coordination, medical/mental health, academic and all other appointments as deemed necessary for the children placed by the agency.
    • Participate in mandatory rotation for 24 hour "on call" to support our clients and families.
    • Participate in group/individual supervision sessions.
    • Represent Ariel Clinical Services by acting as a liaison to local human service agencies and state level agencies, as required/assigned.
    • Monitor the TFC Homes to ensure compliance with rules and regulations and assist in training through regular contact and a minimum of monthly home visits.
    • Conduct full/update Structured Analysis Family Evaluations (SAFEs), as qualified, to determine the ability of foster homes to meet children's needs.
    • Participate in ongoing program evaluation to ensure program effectiveness.
    • Assist in the process to certify foster care homes and place children in foster care homes.

    Required Knowledge, Skills and Abilities


    • The ability to effectively communicate verbally and in written format in both individual and group settings.
    • Ability to be flexible, and able to change direction rapidly.

    • Ability to work independently, prioritize workload, complete a wide variety of tasks in a timely and accurate fashion and direct others, as needed.
    • Basic computer knowledge.
    • Ability to organize and present case material in a straightforward and concise written manner.
    • Knowledge of human development regarding major milestones of development and normal development parameters.
    • Knowledge of family dynamics regarding communication, interactions, and general functioning as well as parenting skills and intervention techniques.
    • Ability to provide reliable, insured transportation for self and others through the community served.
    • Ability to demonstrate consistent and reliable attendance as scheduled and requested.

    Education and Experience

    Required:



    • Bachelor's degree in social or behavioral sciences.

    OR

    • Bachelor's degree in a non-social or behavior science, and,
    • Two years experience supervised by an individual with a MSW or other master's degree in the social or behavioral sciences.

    Preferred:



    • Minimum of one year experience working in Child Welfare or Human Services.
    • Structured Analysis Family Evaluation (SAFE) certification at hire with prior related experience.
    • Experience in customer service leadership role
      OR1 year I/DD or mental health experience.

    Certifications/Licenses

    Required:


    • Valid Regular Drivers License, with acceptable driving record.
    • CORE training
    • CPR, First Aid
    • CPI
    • Informed Supervision
    • BSIT

    Supervisory Responsibilities

    Provides direction to Case Management Aides and Interns, as requested.


    Physical Demands


    • This position requires that the incumbent be capable of bending, sitting, stretching, and/or reaching regularly for periods up to two hours without a break.
    • This position requires that the incumbent have adequate hand and finger dexterity to properly perform the functions of the position.
    • This position requires lifting regularly up to 10 pounds and occasionally up to 40 pounds.
    • This position requires the ability to hear and see at a level which is adequate to ensure client and staff health and safety. Adaptive devices are acceptable.
    • This position will require daily driving of up to one hour per day and occasionally up to eight hours per day.

    Work Environment


    • Exposure to typical climate-controlled office environment when in the office.
    • Exposure to all climates and weather conditions while working in the field.
    • Exposure to bloodborne pathogens and infectious disease is possible but limited.
    • Exposure to hazardous chemicals is possible but limited.
