    As an Admissions Coordinator, you will play a pivotal role in our facility by overseeing the seamless transition of residents into our care. Utilizing your clinical expertise and compassionate nature, you will guide families through the admissions process, providing support and reassurance during what can be a challenging time.

    Responsibilities:

    • Collaborate with healthcare professionals, including physicians, nurses, and therapists, to ensure a comprehensive understanding of each resident's needs and care requirements.
    • Conduct thorough assessments to determine the most suitable care plan for incoming residents, taking into consideration their medical history, physical condition, and personal preferences.
    • Serve as a liaison between residents, families, and our interdisciplinary team, fostering open communication and addressing any concerns with empathy and professionalism.
    • Coordinate with insurance providers and external agencies to facilitate the financial and logistical aspects of admissions, ensuring a smooth and efficient transition for all parties involved.

    Qualifications:

    • Registered Nurse (RN), Licensed Practical Nurse (LPN), or Certified Occupational Therapy Assistant (COTA) preferred.
    • Prior experience in healthcare admissions, case management, or a related field
    • Strong understanding of medical terminology, patient assessment, and care planning
    • Exceptional communication skills and the ability to build rapport with diverse individuals.
    • Proficiency in navigating electronic health records (EHR) and other relevant software applications.
