    Job Summary

    The Registered Nurse in Home and Community-Based Services provides personalized care to patients in their homes, as prescribed by physicians or authorized by state/program regulations. This role requires coordination with healthcare professionals to ensure continuity of patient care and maintain infection control standards.

    Key Responsibilities

    1. Provides high-quality clinical services within scope of practice, adhering to infection control standards and coordinating with the patient's care team.
    2. Completes clinical nursing assessments in accordance with federal and state program requirements and payer sources.
    3. Evaluates patient eligibility and medical necessity for services, ensuring compliance with agency policy and payer sources.
    4. Develops and revises individualized plans of care and service plans, collaborating with community providers and ensuring plan frequency and duration meet patient needs.

    Requirements

    To be successful in this role, you will need:

    • A minimum of one year experience as an RN (preferred)
    • Current RN licensure in the state of practice
    • Current drivers license, vehicle insurance, and access to a dependable vehicle or public transportation
    • Current CPR certification

    Estimated salary: $65,000 - $85,000 per year (dependent on location and experience).
