- Collaborate with healthcare professionals, family members, and caregivers
- Work in various settings including home visits, skilled facilities, and telehealth
- Master's Degree or equivalent experience with NP licensure
- Current RN and NP licensure, board certification in a Matrix approved specialty
- BLS, ACLS or CPR certification, minimum of 1 year NP experience
- Strong communication skills and flexibility with change
- Valid driver's license and automobile insurance, ability to travel
- Travel to meet patients in their living environment

About the Position:
Matrix providers conduct comprehensive health assessments by reviewing a member's health, medical history, social environment, and other risks during visits that can last up to an hour. This in-depth assessment allows us to improve the quality of care and address multiple care gaps in one visit.

Type: PRN/Part Time - Flexible-Nurse Practitioner
PAY: $90.00 or $95.00 (based on county) per completed Assessment
Location: Home, SNF, Telehealth, and other community environments in Cillicothe, Ohio
Hours: Days, Evening, Nights, weekend, Flexible Scheduling
Benefits: PRN/Part Time providers receive 401(k) and limited Voluntary Benefits, Mileage Reimbursement plan
