    POSITION:


    To assist residents in a personal care environment with various daily functions and provide routine care, which may include transfers, assistance with walking, personal hygiene, nutrition and assistance with meals, passing medications, encourage participation with activities and other daily tasks in a geriatric environment.

    An outline of essential duties of the position is listed below.


    To assist in planning, organizing, developing and directing Nursing Service in accordance with current applicable federal, state and local standards, guidelines and regulations, and as may be directed by the Department Director or Administrator, to assure that the highest degree of quality resident care can be maintained at all times.


    RESPONSIBILITIES:

    • Provide hands on assistance to residents, such as bathing, showering, dressing,


    grooming (includes nail care, oral hygiene, personal hygiene), ambulation, transferring, monitoring and/or passing of medications, bed making, minor treatments, incontinency care, as directed by charge personnel.


    • Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the facility.
    • Responsible to do assigned cleaning duties to identified areas.
    • Make independent decisions regarding resident needs.
    • Will assist with the development of resident care plans and discharge plans if directed.
    • Check residents as needed to assure that prescribed treatment is being properly administered by direct care staff and to evaluate their physical and emotional status. Record findings in the resident's record.
    • Chart notes in an informative and descriptive manner that reflects the care provided as well as the resident's response to the care, including, but not limited to, incidental daily occurrences, such as resident outings, visitations, etc.
    • Will accept and perform duties as assigned on task list.
    • Forward new diet orders, and/or diet changes to the dietary department.
    • Responsible for notification to pharmacies for refills on prescriptions or notification to physician for expired orders.
    • Prepare necessary lab sheets for resident blood draws.
    • Verify the identification of the resident before administering medication, treatment, etc.
    • Assist residents with menu selections as needed.
    • Set an example of Rouse Estate Care Values.
    • Checking and recording vitals.

    Administer professional service such as:

    applying and changing dressings/bandages, packs, colostomy and drainage bags, sputum, catherization, urine specimens, massage and exercises, care of the dead/dying, etc., in accordance with established policies and procedures.


    • Communicating and change in resident's functioning and call for 911 assist to hospital if necessary.
    • Assure that all personnel follow established infection control and aseptic techniques at all times and when isolation procedures become necessary.
    • Responsible for checking on residents throughout shift.
    • Monitor budgetary aspects of resident care supplies and supplies needed for duties.
    • Responsible to maintain medication keys on is/her person during assigned shift and to transfer keys at beginning and end of shift as needed.
    • May be asked to complete other duties as assigned.
    • Monitoring and passing medications after medication orientation/completion.
    • Monitor medication cart and resident medical files as per regulation requirements, including completing audits.
    • Attend required meetings and report pertinent information back to staff per Administrator/Supervisor.
    • Actively provide education to staff on Medication Administration to new staff, recertify staff and education as needed.

    INITIATIVE:

    • Attend and participate in your professional associations activities and programs, etc., to assure that you keep abreast of current regulations and guidelines, as well as professional standards and make recommendations on changes in policies and procedures to the Administrator/Supervisor.
    • Check resident charts for specific treatment, medication orders, schedules, etc., daily. Complete double check of orders entered for accuracy.
    • Review resident care plans for appropriate resident goals, problems approaches, etc., to assure appropriate nursing care is being rendered, and add to care plan as resident condition warrants in coordination with the interdisciplinary team as/if directed.
    • Perform routine charting as required.
    • Complete medication cart audits as required.

    PRODUCTIVITY:

    • Meet with Administrative/Nursing personnel, on a regularly scheduled basis, to assist in identifying and correcting problem areas, and/or the improvement of services.
    • Make written and oral reports/recommendations to the RN Supervisor concerning resident care for shift.
    • Assist in standardizing the methods in which work will be accomplished.
    • Review resident care plans, EMAR, treatment plan, etc., at least monthly or as directed by RN Supervisor, and make recommendations to RN Supervisor.

    KNOWLEDGE OF WORK:

    • Assist in developing and maintaining nursing objectives and standards.
    • Interpret department policies and procedures to personnel, residents, visitors, family members etc., as necessary.
    • Ensure that resident's rights to fair and equitable treatment, self-determination, individuality privacy, property and civil rights, including the right to wage a complaint, are followed by all department personnel.
    • Maintain confidentiality of all pertinent resident care information to assure resident rights are protected.
    • In cases of death, follow procedure outlined in our established policy.
    • Evaluate emergencies and determine emergency measured needed. Following emergencies, identify any areas of further education needed.
    • Fill out and complete accident/incident report forms on any resident accident/incidents and chart such information on the resident's notes as outlined in Policy and Procedure if responding to the incident.
    • Attend and participate in-service meetings.
    • Take TPRs, blood pressures, etc. and chart as required.
    • Prepare/administer medications as ordered by the physician
    (Note:

    Medications set up by one nurse may not be administered by another medication certified technician.)


    • Answer telephone inquires as outlined in our established policies and procedures.
    • Complete required recordkeeping forms/charts in the resident's chart. File in the resident's chart and/or forward to RN Supervisor, as appropriate.
    • Order prescribed mediations, supplies, and equipment as established by policies and procedures from appropriate agencies, as directed.
    • Review physician's orders for completeness. Update appropriate practitioner if clarification is needed.
    • Transcribe physician's orders to resident medex treatment plans, etc., to assure that they remain current.
    • Requisition and arrange for diagnostic and therapeutic services, as ordered by the physician, either within the facility or by outside arrangements, in accordance with established policies and procedures.
    • Assure that narcotic records are current at all times.
    • Notify physician changes or resident's condition after assessment. Ensure this is documented.
    • Review eMAR for completeness of information, accuracy in the transcription of physician orders, and adherence to stop order policies.
    • Report all discrepancies of physician's orders, diet, charting, etc., to the RN Supervisor.

    COOPERATION:

    • Develop and maintain a good working rapport with interdepartment personnel, as well as other departments with in the facility to assure that the department services and activities can be properly maintained to meet the needs of the residents.
    • Coordinate Nursing Services with other departments.
    • Communicate regularly with Leadership/Administrative staff to keep accurate understudy of any changes or problems solving needs that affect all shifts.
    • Work with the facility's consultants as necessary and implement recommended changes as directed by the Administrator/RN Supervisor.
    • Gives/receives nursing shift report upon reporting in and ending shift duty hours if applicable.
    • Assist in orientation of newly hired employees as well as supervise and instruct other personnel.
    • Orient new residents/family members as necessary.
    • Provide training to new shift hires as directed.
    • Educate and train Direct Care staff through Medication Administration program.
    SAFETY/


    HOUSEKEEPING:

    • Assure that department personnel, residents, visitors, etc., follow established policies and procedures to include safety rules and regulations, at all times.
    • Assure that stock levels of medication, medical supplies, equipment, etc., are maintained at adequate levels at all times. Requisition equipment and supply needs as required.
    • Evaluate emergencies and determine emergency measures needed.
    • Assure that the facility is maintained n a clean and safe manner for resident comfort and convenience by assuring that necessary equipment and supplies are maintained to perform such duties/services.
    • May perform minor housekeeping duties around the facility.
    ATTENDANCE/


    PUNCTUALITY:

    • Report on and off duty at break times and meal times.
    • Report for duty on all scheduled days including weekend rotation.
    • Must arrive on time and accurately log time in/out in appropriate computer software system.
    • LPN schedule is based on 10:00 am to 6:00 pm shift, unless otherwise approved or directed by Administrator/RN Supervisor.

    SUPERVISION:

    • Accountable for delivery of care and outcomes through
      LPNtraining.
    • Exercises independent judgment in evaluating the performance of Direct Care Staff during Medication Administration training.

    WORKING CONDITIONS:

    • Works in well-lighted/ventilated work areas, as well as throughout the nursing service areas.
    • Sitting, standing, bending, lifting and moving intermittently during working hours.
    • Subject to frequent interruptions.
    • Involved with residents, family members, personnel, visitors, etc. under all conditions/circumstances.
    • Subject to hostile and emotionally upset residents, family members, etc.
    • Communicates with direct care personnel, and other department supervisors in a respectful and clear manner.
    • Willingness to work beyond normal working hours, and in other positions temporarily, when necessary and approved by RN Supervisor/Administrator.
    • Attend and participate in continuing educational programs as directed.
    • Subject to falls, burns from equipment, infectious diseases, substances, odors, etc., throughout the work day.
    • Be economical and careful with supplies, equipment, etc.
    • Follow all of our established policies and procedures, to include nursing care procedures, safety regulations, personnel policies, etc., to assure that quality resident care can be maintained.

    QUALIFICATIONS:

    Education:
    A. Must be a graduate of an approved
    LPNprogram and possess a valid PA nursing license.


    Experience:
    A. 3-5 years of experience in a hospital, skilled nursing care facility, or other related medical facility required.

    B. Experience must demonstrate strong clinical background.

    C. Experience with pharmaceuticals preferred.

    D. Training in rehabilitative and restorative nursing practices a plus.


    Specific requirements:
    A. Must have six additional education hours annually in dementia care approved by the Department of Human Services.

    B. Must be able to read, write and speak the English language.

    C. Possess the ability to make independent decisions when circumstance warrants such action.

    D. Possess the ability to deal tactfully with personnel, residents, family members, visitors, and government.

    E. Knowledgeable of nursing and medical practices and procedures.

    F. Willing to work harmoniously with professional and non-professional personnel.

    G. Ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, and policies.

    H. Maintaining the care and use of supplies, equipment, etc., of the nursing service department.

    Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents based on whatever cognitive level they are currently functioning.

    J.

    Possess the ability to seek out new methods and principles and be willing to incorporate them into existing nursing practices.

    Must be willing to accomplish tasks in a manner that is consistent with the mission, philosophy and personnel policies outlined by the management of the Suites of Rouse.

    L. Must adhere to uniform requirements.

    M. Must be comfortable with geriatrics and comfortable in assisting them with personal care needs.

    N. After first six months of employment, must begin certification to become Medication Administration Trainer through DHS.


    PHYSICAL REQUIREMENTS:
    A. Must be able to move intermittently throughout the work day.

    B. Must be able to lift up to 40 pounds.

    C. Must be able to speak the English language in an understandable manner

    D. Must be able to cope with mental and emotional stress of the position.

    E.

    Must possess sight/hearing senses, or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met.

    F. Must function independently, have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies.

    G. Must be in good general health and demonstrate emotional stability.

    H.

    Must be able to relate to work with ill, disabled, elderly, emotionally upset, and at times hostile people within the facility.

    I. May be required to lift residents, medical equipment, supplies, etc.
