    Job Description

    We are looking to fill a position for a dedicated Registered Nurse specializing in Hospice for After Hours and Weekends. The work schedule is from Friday at 4:30 pm until Monday at 8:00 am.

    • Capable of assessing patient needs over the phone until an in-person visit is arranged.
    • Prompt response to urgent calls from patients and families during afterhours.
    • Conducts the initial nursing assessment to determine hospice service eligibility within forty-eight hours of referral, if allocated.
    • Proficient in conducting afterhours visits such as admissions, end-of-life visits, recertification visits, and regular visits.
    • Recognizes the physical, emotional, psychosocial, and environmental needs of the patient and their family, with reassessments done at least every fifteen days.
    • Records issues, relevant objectives, interventions, and the patient/family's response to hospice care.

    Experience Requirements

    • Minimum of one year of clinical experience.

    License Requirements

    • Active RN license in the practicing state along with one year of clinical experience.
    • Current CPR Certification.
    • Valid driver's license, appropriate vehicle insurance, and access to a reliable vehicle, or access to public transportation.

    Additional State-Specific Requirements

    • For TN state: No extra state-specific prerequisites.
