Job DescriptionThe Care Manager will oversee patients' needs and coordinate care with providers and other health care professionals.
Duties and Responsibilities
Enroll eligible patients in care management program
Communicate with patient and their families (with consent) to develop actionable and achievable care plans
Monitor and adjust patient care plans based on changing needs and conditions
Coordinate referrals to health care professionals as defined by best practice protocols
Participate in interdisciplinary treatment team meetings to assure care plans meet all of patient needs
Qualifications
Bachelor's degree in nursing or related field preferred, or minimum 2 years of clinical experience required
2 years of care/case manager experience required
Ability to collaborate effectively with physicians and other health care professionals
Experience with EMR systems – eCW preferred
Registered Nurse preferred.
