- Impact people's lives by conducting Adult/Geriatric Assessments, reviewing medical history, providing diagnoses and treatments, offering health education, referring to physicians and case managers, following up, and maintaining proper documentation as per Matrix guidelines
- Collaborate with physicians, case managers, social workers, family members, key caregivers, and other medical staff as needed
- Partner with Primary Care Physicians for patient education and follow-up actions
- Establish and uphold relationships with a national network of advanced practice providers
- Services may be rendered in various settings including Home Visits, Skilled Facility Visits, and Telehealth Visits
- Master's Degree or equivalent experience and successful NP licensure completion
- Master's Degree or equivalent experience and satisfactory completion of PA licensure
- Current RN and NP/PA licenses in the state of practice, inclusive of prescription authorization or eligibility for such authorization
- Board certification by NCCPA, AANP, ANCC, or AACCN in a Matrix-approved specialization
- Valid BLS, ACLS, or CPR certification
- Minimum of 1 year's experience as a Nurse Practitioner/Physician Assistant with the ability to travel within or outside the state
- Proficient in computer operations and familiar with EMR software
- Excellent verbal and written communication skills with patients, clients, and colleagues
- Positive, sociable, team-oriented attitude with adaptability to change
- Ability to travel with a valid state driver's license, car, and adequate automobile insurance for the state of residence

About the Position:
During a visit that can extend up to one hour, Matrix providers assess a member's current health status, medical background, medication adherence, social context, and other potential risks. This process offers valuable insights into an individual's overall well-being that are often missed during regular office visits. The Matrix Comprehensive Health Assessment aims to enhance care quality and address multiple care gaps in a single session.

Matrix providers engage in Adult/Geriatric Assessments, medical evaluation, diagnosis, treatment, health education, physician recommendations, case management referrals, follow-up procedures, and accurate documentation following Matrix standards and procedures
Nurse Practitioner/Physician Assistant - PRN – Home Risk Assessments
Type: PRN-Flexible-Nurse Practitioner/Physician Assistant
Location: Home, SNF, Telehealth, and various community settings in Riverhead, New York
Work Schedule: Days, Evenings, Nights, weekends
Compensation: $90.00 or $115.00 per completed Assessment based on county, 25 hours or more/month-Flexible Scheduling
Benefits: PRN providers do not receive benefits except for limited Voluntary Benefits
Mileage Reimbursement provided for all miles when completing 20 or more assessments/month and No-show payment for 10 or more scheduled assessments/month
Matrix Provider – What to Anticipate:
THIS POSITION IS NOT FULL-TIME – It offers PRN/COVERAGE with flexible hours
