    Clinical Dietitian for Long Term Care Hospitalized patients with interest in Memory Support Care.

    Lead the Nutrition Care Process and workflow elements, which includes nutrition screening, nutrition assessment, nutrition diagnosis, nutrition intervention/ plan of care, nutrition monitoring and evaluation, and discharge planning and transitions of care.

    • Plan, organize, implement and evaluate nutritional care for all patients according to established guidelines
    • Assure the nutritional needs of the patients are being met in accordance to established guidelines
    • Communicates with Health Care Proxy when enacted or per patient request.
    • Perform monthly or quarterly and annual assessments of LTC patients, including MDS (Minimum Data Sheet) assessments.
    • Perform initial assessments and follow-ups of patients on RSU and MCCU, including MDS for RSU
    • Utilize current Malnutrition Guidelines to assess patients for malnutrition including NFPE, hand grip, and reference to the standards of practice.
    • Prioritize High risk patients as indicated in guidelines.
    • Determine short and long term goals and objectives with focus on Patient-Centered Care.
    • Responsible for patient menu development, analysis, implementation and revision
    • Responsible for training and education for staff, employees, residents/families and the health care team as appropriate
    • Participate in patient care conferences and other interdisciplinary meetings
    • Conduct tray assessments and meal rounds to assess intake adequacy, tray accuracy, diet compliance, food quality and resident satisfaction
    • Conduct patient visitations and enter preferences into computer
    • Calculate calorie counts and documents in the medical record
    • Maintain monthly weight records for assigned floors and address concerns for those that are having significant weight loss with appropriate team members
    • Participate in department improvement projects
    • Work with physicians in providing quality nutrition recommendations
    • Assist the Nutrition Assistants with training, education and coverage as needed
    • Perform clerical tasks in the diet office as needed
    • Serve as a resource/mentor to the team, new employees, and students/interns. Educate allied disciplines: nursing, physicians and other staff as necessary.
    • Seek opportunity for professional development by attending internal and external trainings, educational programs, seminars and conferences to maintain clinical skills
    • References include: The Nutrition Care Manual and the Evidence Analysis Library (EAL) and Diet Manual.
    • Meet all requirements as outlined by regulatory and licensure standards
    • Assist in the development/revision of policies and procedures
    • Assure individual competency and demonstrates same in the provision of all aspects of Medical Nutrition Therapy.
    • Maintains good communications and works cooperatively with all team members of the department and personnel within hospital.
    • Familiar with The Academy's Scope of Practice for the RDs, Standards of Practice and Standards of Professional Performance for RDs along with the Code of Ethics.

    Performs other duties as assigned

    • Registered Dietitian (RD) and Licensed Dietitian/Nutritionist (LDN) required
    • Computer skills.

    1. Meditech Expanse and Computrition experience a plus
    2. Experience in Long Term Care or strong interest in working with elders.
