We are looking for a primary care physician with leadership and mentoring skills and a commitment to evidence based medical practice, who is comfortable managing complex patients and dedicated to personable patient care, and a collegial, team-based work environment.
The unique model of our health system integrates twelve health centers, including five health centers providing primary care, behavioral health and walk-in care, two dental clinics, a vision center, four school-based health clinics and a site within our local recreation center.
Our health centers are certified Patient-Centered Medical Home sites.We strive to provide evidence-based care that is accessible and affordable to all (based on a sliding fee scale) and results in healthier outcomes for our communities.
