- Patient Care: Assist patients as needed with lifting and transferring
- Communication: Maintain good relationships with the patient, the patient's family, and staff. Reports any changes to the patient's medical and living conditions to the supervisor or the case manager.
- Minimize Patient Risk: Ensure the living environment is healthy and safe. < li>Policies: Completes all documentation following agency protocol and Medicare/Federal guidelines . < li>Operations : Attend in-service training and mandatory agency meetings.
