    Description
    Job Details:
    • Position: Nurse for Assessment and in-office (Full Time Position)
    • Shifts: Morning/Afternoon.
    • No nights or weekends
    Job Responsibilities:
    • Evaluates patients referred to home health care services
    • Interviews patient and family to determine appropriateness for services
    • Obtain referral documentation from physicians and insurance
    • Upon completion of the assessment, all cases must be transmitted and completed paperwork must be submitted
    • Abides by and promotes Agency administrative policies.
    • Attends meetings and training as required (webcam or in-person).
    • All consents and original intake assessment documents must be submitted within 5 days via mail or drop off at a health home location.
      • Assists in agency continuing education and inservice program
    Education Requirements:
    • Graduate of 2-4-year accredited nursing program.
    • Bachelor's Degree in Nursing or advanced degree is preferred.
    • Active and unrestricted NY State Registered Nurse (RN) License.
    • Knowledge of current community health practices for the frail adult population, behavioral health, and cognitively impaired seniors and disabled including knowledge of physical health, aging, and loss, appropriate support services in the community, frequently used medications and their potential negative side-effects, depression, challenging behaviors, Alzheimer's disease, and other disease-related dementias.
    Experience And Skills Required:
    • Minimum of three to five years of demonstrated clinical and supervisory experience.
    • Solid assessment and documentation skills.
    • Ability to effectively communicate verbally and in written formats at all levels.
    • High level of professionalism with the ability to handle sensitive and confidential information.
    • Meticulous attention to detail and strong organizational skills.
    • Ability to use computerized systems such as Microsoft Outlook, Microsoft Word, Excel.
    • Ability to read and interpret documents such as safety rules
    • Ability to solve practical problems
