As a Licensed Practical Nurse in the Medical Group practices, you will be providing essential nursing care to patients, families, and caregivers. Working closely with a multidisciplinary care team, your role involves advocating for patient and family health, in line with the patient-centered approach. Performing tasks within the scope of case finding and health coaching, you will support the overall delivery model of care.
