    We are currently seeking a skilled OT to provide comprehensive therapy services in patients' residences.

    Job Summary and Duties:


    As an Occupational Therapist (OT) you will be responsible for evaluating, planning, and administering comprehensive occupational therapy modalities to patients in their place of residence.


    Your duties and responsibilities will include:

    • Performing accurate and comprehensive assessments.
    • Obtaining physician orders for certification and recertification of home occupational therapy.
    • Communicating changes in patients' clinical status to healthcare team members and Agency office staff.
    • Coordinating care planning with physicians, patients, families, and healthcare team members.
    • Delivering occupational therapy modalities appropriate to the patients clinical status and needs.
    • Educating patients, families, and caregivers in appropriate occupational therapy modalities.
    • Synthesizing data from physical examinations to make clinical judgments regarding patient management.
    • Identifying and addressing risks to patient safety and implementing interventions to reduce those risks.
    • Assessing the need for durable medical equipment and adaptive equipment.
    • Developing home exercise programs tailored to the patients clinical status, needs, and age.

    Professional Requirements:
    To be successful in this role, you must meet the following requirements:

    • Valid OT license in good standing with the State.
    • Graduate of an accredited Occupational Therapy curriculum.
    • Two years of work experience preferred, with prior home healthcare experience preferred.
    • Current BCLS certification.
    • Current health certificate/physical examination and TB testing.
    • Ability to relate positively and favorably with patients and staff.
    • Excellent oral and written communication skills, along with good documentation skills.
    • Knowledge of occupational therapy modalities and scope of occupational therapy practice in the State of practice.
    • Flexible, organized, and able to exercise sound judgment.
    • Effective communication in English; additional languages preferred.

    Additional Requirements:

    • Adheres to dress code and legal practice standards.
    • Completes annual education requirements.
    • Maintains patient confidentiality at all times.
    • Demonstrates effective time management and organizational skills.
    • Attends staff meetings and participates in departmental inservices.
    • Ensures compliance with policies and procedures regarding department operations, safety, and infection control.
    • Participates in the organizations performance improvement and continuous quality improvement (CQI) activities.
    • Practices in accordance with the professional and Agency code of conduct.
