Description
You will manage supportive services for 40+ clients through the Transformational Homelessness Response grant and provide services to increase access to social determinates of health for people who are unhoused and currently in an acute care setting .
This includes providing supportive services/ case management to the clients, managing grant paperwork, entering HMIS documentation, and partner development and maintenance.
This role includes skills that include collaborative work with all MHP teams, case management skills, supportive counseling skills, communication skills, documentation skills, organizational skills and partnership skills.
Other housing related tasks as needed.Job Profile:
Are you passionate regarding the homeless population, building relationships with those experiencing homelessness, engaging them and connection them with resources, services, and housing? Do you have experience helping clients with complex barriers and managing their cases and managing grant documentation? Do you want to be out in the community working directly with people experiencing homelessness, where they are, to provide education, connection and assistance?You will manage supportive services for 40+ clients are accessing acute services and experiencing homelessness.
The Transformational grant is the opportunity to engage clients struggling to access services that will stabilize their housing and overall wellness needs.
You will manage supportive services for 40+ clients who live at our partnership location the Suites and does client care for the transitional housing program at the Inn Between.
This includes providing supportive services/ case management to the clients, managing grant paperwork, entering HMIS documentation, and partner development and maintenance.

Bachelor's Degree in Psychology, Social Work, Counseling, or related field preferred or one-year experience in the behavioral health field for non-related Bachelor's degree Previous experience working with individuals who have been unhoused and/or individuals in an acute care setting is helpful but not required We want people who can work independently and also on a team.
Will work with community partners around clients. Must have current CO Driver's License and a safe driving record Office environment and in the community. Includes outreach and engagement with clients who have struggled to connect with services and housing.This position will close 5/26/2024 Qualifications Mental Health Partners began in 1962 as a clinic, received non-profit status in 1964, and was federally designated as a comprehensive community mental health center in 1971.
Since then, we have expanded to meet the growing needs of the community in both counties, with locations in Boulder, Broomfield and Longmont.
We are the only agency in Boulder and Broomfield counties that provides comprehensive psychiatric services to county residents regardless of their ability to pay.
People served by Mental Health Partners generally do not have access to services in the private sector because they cannot afford to pay the significant fees, and most do not have insurance.
Mental Health Partners has 24 hour emergency psychiatric services and outpatient offices for infants, children, adolescents, adults, and families. Many programs and services are available including school-based programs, substance abuse treatment, rape crisis counseling, advocacy, and education. Mental Health Partners offers bilingual/bicultural services and an in-house pharmacy.It is estimated, that of the 360,000 residents of Boulder and Broomfield counties, over 60,000 individuals suffer with a form of mental illness.
This number is only compounded when family, friends, and co-workers are affected by the disabling conditions of a person who is struggling with a mental illness.
Annually, Mental Health Partners serves over 17,000 individuals and our Emergency Psychiatric Services Team responds to 10,000 calls. We serve more women than men and thirty-six percent of our clients are multicultural.
