- Support the In-Home Special Benefits Program's initiatives across growth, operations, and new ventures.
- Conduct outreach activities to identify, schedule, and coordinate In-Home Services for eligible members.
- Collaborate with vendors and internal stakeholders in expanding the programs reach and depth.
- Develop and maintain a comprehensive scheduling system to manage home visits, ensuring optimal coverage and resource utilization.
- Maintain accurate up-to-date records of member information.
- Facilitate health coaching during member interactions, providing guidance on wellness, nutrition, and overall health improvement.
- Identify and report any environmental, physical, mental, or emotional distress that the member might be experiencing.
- Provide community resources and company services that may benefit the member.
- Associates Degree or Bachelor's degree
- 0-1+ years' experience in Project Management, Social Services, or Healthcare
- BILINGUAL - Ability to multi-task and effectively communicate in Mandarin and/or Cantonese
- Extensive experience with Microsoft Office Suite (Word, PowerPoint, Excel)
- Comfortable with working alongside Senior Citizens
- An authentic passion for improving the lives of underserved patients and advancing value-based care.

- Our organization follows a hybrid work structure where the expectation is to work both in office and on the field. After an initial 30-day training period, you will be eligible to work remote on non-field days.

- The total compensation target pay range for this role is: $20 - $28 per hour. The salary range represents our national target range for this role.
