    Founded in 1875, our agency has grown over the years to address most of the challenges children, adults, families, and communities face across the lifespan, including substance use, child abuse and neglect, domestic violence/sexual assault, human trafficking, homelessness, juvenile justice, mental health, neighborhood distress, and poverty.

    Today, we offer 42 programs from 20 locations in east central Nebraska and southwest Iowa.

    SUMMARY DESCRIPTION OF PROGRAMHeartland Bridges houses two programs, BridgesTransitional Living and Bridges Crisis Care.

    Transitional Living is a short-term transitional housing program enriched with supportive services designed to assist people with a diagnosed mental health or co-occurring condition while they transition to stability in the community.

    Crisis Care is a residential stabilization service designed for individuals who need a safe, secure environment for 3-5 days while stabilizing from a crisis before returning to the community where they live.

    SUMMARY DESCRIPTION OF WORKThe Case Manager I serves as a direct care staff that provides intensive case management to Bridges transitional living program participants.

    This role coordinates supportive services designed to meet the needs of the client while working with them to develop a plan for permanent housing.

    Decision making, problem solving, and highly developed interpersonal skills are critical.


    Compensation:
    between $15.29 and $18.23 per hour, (determined by years of experience)

    part time (to be discussed during interview)MINIMUM QUALIFICATIONSHigh School diploma requiredOne year experience in a social service settingValid driver's license and acceptable driving recordPREFERRED QUALIFICATIONSBachelor's DegreeMental health or substance abuse treatment experienceOne-year experience in a 24-hour care settingESSENTIAL DUTIES AND RESPONSIBILITIESResponds to crisis care referral calls, assesses individual by gathering information and provides appropriate and accurate application verdict, maintaining strict confidentialityCompletes enrollment/intake on eligible transitional and crisis clientsDocuments client activity, service plan progress and needs per established record keeping proceduresProvides problem solving and crisis intervention services to empower clients to develop self-sufficiencyEnsures required information is collected, entered in prescribed formats, and reported in a timely mannerProvides program information and referral services to clientsMonitors client self-administration of medication and follows procedures of recordingWillingness and ability to obtain/maintain certification to administer non-parenteral medications in the state of IowaPsycho-educates clients on life, social and coping skillsAssists facilitation of in-house client activities, education, and support groupsDemonstrates knowledge of current community and HFS programs and resources to meet client needs, including providing connections to mainstream benefits, income support services and Integrated Health Home servicesUtilizes trauma-informed care, person centered and culturally competent principles in providing services to clientsEvaluates efficiency and effectiveness of community service providers on an ongoing basis to ensure clients are receiving quality careOrients clients to Bridges protocols and addresses client non-compliance as necessary to create a safe and secure environment for all residents and staffConducts client rooms safety checks and room searches per program protocolResponsible for maintenance and upkeep of facilityAble to work both independently, with little supervision, and as a part of a teamAlways displays a courteous and caring attitude to the clientele, volunteers, and visitors of the AgencyAbides by all specific program and Agency procedures, policies, and requirementsCooperates and collaborates with program area staff, volunteers, and other Agency staffDevelops personal and program related skills through participation in internal and external training opportunities including printed material and audio and/or visual mediaCreates, maintains, and shares as appropriate a dynamic self-care planEssential functions of this job are to be performed on company physical work sitePerforms other program related duties as assignedEqual Opportunity Employer/Protected Veterans/Individuals with DisabilitiesThe contractor will not discharge or in any other manner discriminate against employees or applicants because they have inquired about, discussed, or disclosed their own pay or the pay of another employee or applicant.
