Job Summary
The Care Coordinator will be responsible for working with neonate, adolescent, adult,and geriatric patients,families medical staff and community agencies to determine
the appropriate setting of care and appropriateness of hospitalization as well as
to assess post hospital care needs
- Masters of Social Work (MSW) OR Bachelor's degree in health-related field.
- Three (3) years of experience in case management social services or related field.
- Certified Case Manager
