The Hospice Admission Nurse (RN) provides compassionate, patient-centered care by utilizing critical thinking skills to perform comprehensive assessments, develop individualized plans of care, and deliver high-quality hospice services.
Responsibilities
- Conducts comprehensive assessments of patient and family needs—physical, emotional, social, and spiritual—and collaborates with the interdisciplinary team (IDG) to develop and implement the plan of care.
- Maintains timely and accurate documentation of patient status, interventions, and goals in compliance with agency protocols and regulatory requirements.
