As the Home Health Patient Care Manager Assistant, you are responsible for assisting the Patient Care Manager in coordinating a team of staff to assure the continuity of high-quality care to patients assigned to the team's area.Responsibilities
- Ensures that the team is committed to providing quality care in an efficient manner in accordance with physician's orders and State/Medicare guidelines
- Prepares patient list and necessary documents for weekly case conferences
