Seeking a Part-time Home Health/Home Care Nurse Manager in a fast-paced Home Care agency, serving clients in Hamilton, Robbinsville, Princeton, Trenton, and neighboring towns.

Job duties:
Support and assist DON in clinical operations including compliance and quality improvement.
Provide thoughtful care to clients & patients based on personalized Plan of Care.
Case management- New client assessment, Plan of care, follow-up, and re-assessment
Monitor and track changes in the client/patient's conditions and needs, and keeps the Director of Nursing and any other appropriate personnel informed
Proactively updates client/patients' family on plan of care, health changes, and concerns
Participates in field staff training and orientation
Driving to client homes as needed
Clinical requirements (including but not limited to):
High-tech nursing
IV insertion and Infusion Experience
Blood sugar testing
In-home blood draws and injections
Elder care management
Medication setup, administration, and oversight
Tube, drain, and bag maintenance
Vital signs
Wound care
Vaccine Administration
Requirements:
New Jersey RN license
1 year Home Care Nursing Experience
Valid NJ driver's license
Valid car insurance
