    Hours: 8 working hours between 6:30 am to 5:30 pm Monday- Friday
    Location: Seymour Johnson Air Force Base, North Carolina


    Qualifications:
    • Degree/Education: Master's degree in Social Work (MSW) or Registered Nurse. Graduate from a school of Social Work fully accredited by the Council on Social Work Education (CSWE).
    • Experience: Experience in care coordination, and discharge planning.

    Job Duties and Responsibilities:The duties include but are not limited to the following:
    • Coordinate appropriate referrals to community and other Government agencies for services.
    • Evaluate medical and educational needs of family members to advise families, dependents, AFPC, and MDG staff on the appropriateness of family member travel as part of the EFMP-Medical program. Continuously monitor local health resource availability.
    • Assess, document, and transmit information about the special education and medical needs of all family members planning to move at the government's expense. These needs include specialized medical care, medications, equipment, or any early intervention or related services required or recommended for a child, typically specified on an Individualized Family Service Plan (IFSP) or Individual Education Program (IEP).
    • Independently assess the biopsychosocial needs of patients and their family members.
    • Use medical and mental health diagnoses and disability terminology. This includes acute, chronic, and traumatic illnesses/injuries, common medications, and medical terminology.
    • Assist AFPC in assigning sponsors to areas where family members' medical and/or special education needs can be accommodated, provided there are valid manning requirements for the Airmen's grades and specialties.
    • Provides training to installation personnel and medical staff; and consultation as needed to support the implementation of EFMP-M base-wide. Completes all required electronic medical record training, MTF-specific orientation, and SNC training programs.
    • Integrally involved in the Family Member Relocation Clearance (FMRC) process. Ensures all FMRC requirements are implemented by all EFMP-M staff. Coordinates the enrollment process and travel screening for all active-duty family members and travel screening upon request to the families of DOD civilian-sponsored assignments going to an overseas base.
    • Follows applicable local MTF/AF/DOD/DHA instructions, policies, and guidelines.
