    PATIENT INFORMATION Patient Demographics:
    Adults with endocrine disorders including diabetes Patient Volume/Census: To be built collaboratively; Diabetes Clinic support in place

    Case Mix:
    Endocrine disorders with a focus on diabetes care FACILITY INFORMATION EMR System: Not specified

    Specialty Backup:
    Yes collaboration with APRN and RN

    Diabetes Educator Support Staff Available:
    Full clinic support team, including clinical educators and nursing PRIVILEGES & COMPLIANCE

    Hospital Privileges Required:
    No (100% outpatient)

    Credentialing Timeline:
