- Assist in resident/patient/family education and re-examine, re-evaluate and modify care plans as necessary, including maintaining proper documentation in resident charts
- Prescribe medication as necessary
- Review all diagnostic testing results and make recommendations to findings
- Ensure Encounter forms are completed accurately and in a timely manner
- Educate and supervisor rotating medical and physician assistant students
- Participate in on call monthly/weekend medical coverage
- Maintain continual medical education credits
- Two (2) or more years of experience in a sub-acute or long term care facility administering medical care to residents
- Graduate of an accredited physician assistant or nurse practitioner program
- Clean and active license as an NP or PA New York State, with prescriptive privileges
- Proficient in SigmaCare or another EMR (Electronic Medical Record) software
- Prior experience with the geriatric population a plus
- Fully vaccinated against COVID-19
