    They will also work closely with our ACO participating providers and post-acute care facilities, including facility and physician group management teams.


    About the Role:


    This role is responsible to drive and achieve the performance goals of the ACO at the provider and facility level.

    The Director of Account Management serves as the primary point of contact for developing long-term relationships with ACO participants, providers, and facilities.

    This position provides engagement leadership and is regarded by the client as an expert and trusted advisor.

    The Director of Account Management will play a major role in the ACOs national growth by identifying opportunities within existing participants and possessing the ability to recommend additional areas of value-add for the ACOs participants.


    The Details:
    This is a virtual, work-from-home opportunity.

    In this role, you will be responsible for:
    Delivering exceptional client service on a day-to-day basis, proactively anticipating the clients needsIdentifying and recommending to ACO leadership on areas of growth opportunities for ACO participantsTracking and managing engagement activities to align and achieve the goals of the organizationCoordinating and scheduling partner success meetings, such as Joint Operating Committees and performance reviewsLeading the support of the on-boarding of new clients from business development handoff, Telemedicine handoff, and to go-liveAssisting providers and facilities with the installation, utilization, education, and training of the ACO population health platformEngaging participants to collect and submit required demographic dataEstablishing and engaging participants on their monthly ACO performance reports from the ACO population health platformPreparing monthly performance reports for ACO executive team on accounts that are performing well and underperformingPreparing individualized performance reports for providers and facilitiesWorking with ACO Clinical leadership on preparing monthly/quarterly JOC slides for the participant groupsCollaborating with the ACO clinical team, operations teams, Telemedicine team, and vendors to identify improvement opportunities for ACO participantsManaging other duties as assignedWhat we are looking for:A successful candidate will have a demonstrated track record of a combination of these values, knowledge, and experience:Values :Relationship Building and Maintenance: The ability to create and nourish healthy, strong relationships, as the face of SoundCommunication: The ability to speak, write, and listen clearly and consistentlyCritical Thinking : The ability to be initiative-taking; anticipate needs; ability to make good decisions with incomplete, ambiguous information
