    The CNA/CMT/HHA should have a high school diploma with two (2) years of experience in residential

    setting supporting individuals with developmental disabilities. The CNA/CMT is a direct care level position

    at PBCS. He/she may be a full-time or part-time employee; hours of work include regularly scheduled

    weekday and weekend shifts.

    Responsibilities of the Direct Care giver (C.N.A., CMT, and/or HHA)

    1. Supporting the individuals to achieve personal goals and desires.

    2. Ensuring personal hygiene of the individuals.

    3. Checking and changing individuals' diapers every two (2) hours and PRN.

    4. Meal preparation and cooking.

    5. Completing laundry on each shift, folding clothing and placing neatly in assigned places.

    6. Keeping home in clean and conducive environment.

    7. Participating in the development of Individual Plans (IP) and the daily implementation of the plan.

    8. Doing individuals book-keeping including accounting for monies in the homes.

    9. Reporting changes in skin condition, BM habit and appearance and other health and/or behavior

    concerns to the LPN, Clinical Services Manager (CSM), and RN.

    10. Posting individuals' activity schedules in each residence and following up the schedules to endure

    that they are done in timely manner according to the IP.

    11. Reporting immediately any unusual incident to the LPN on duty, the CSM and/or the

    RN.

    12. Understanding all PBCS policies and procedures, respecting individuals' rights.

    13. Providing individuals with relevant skills and services to enable the individuals live in the least

    restrictive and supervised environment.

    14. CMT is responsible for understanding all the procedures explained in medication

    administration training course and following these procedures in the daily administration of

    medications to individuals (CMTs only).

    15. Knowledge in the use of individuals' medical equipment and making sure that it is in good

    working condition before each use.

    16. Ensuring that transferring of individuals is completed by two (2) staff

    17. Turning and positioning of individuals every two (2) hours or as needed.

    18. Completing BM, menses, vital signs and weight records per shift or as needed.

    19. Completing monthly wheelchair, side rails safety checks and reporting to LPN, CSM and RN if

    faulty.

    20. Completing seizure record after every episode of seizure activity and/or every month even when

    no seizure occurred.

    21. Attending medical and other required appointments as assigned.

    22. Advocating for individuals rights.

    23. Completing all required DDA core training within 90 days of hire, PBCS In-Services and

    meetings.
