- Represent Empyrean Hospice professionally and compassionately
- Conduct spiritual assessments of hospice program patients and families/caregivers, ensuring timely service delivery
- Participate in comprehensive assessments to create individualized care plans addressing spiritual needs
- Collaborate with interdisciplinary team, offering consultation, education, and support on spiritual care
- Deliver direct spiritual care to patients and families/caregivers
- Act as liaison and support for community chaplains and spiritual counselors
- Maintain records of spiritual care services and activities
- Recruit chaplains, counselors, and volunteers, fostering community partnerships and providing education on hospice care
- Arrange funeral or memorial services as needed
- Organize periodic memorial services for personnel, volunteers, and clergy working with the hospice team
- Supervise spiritual care volunteers assisting patients/families/caregivers
- Prioritize ethical business and clinical practice standards
