WellMed is seeking a RN to join our team in Beeville. As a member of the Optum Care Delivery team you'll be an integral part of our vision to make healthcare better for everyone.Responsibilities
- Works with providers and clinic staff to identify patients at high risk
- Suports longitudinal care of the patient with chronic care conditions by assessing health conditions performing medication reconciliation conducting motivational interviewing and self-management goal setting providing patient education creating referrals to appropriate agencies and resources supporting transition of the patient with chronic care conditions from inpatient to outpatient setting by assessing transitional needs performing medication reconciliation establishing and reviewing contingency plan providing patient education assisting post discharge needs such as prescriptions transmutation Durable Medical Equipment appointments coordinating with providers to establish or update plan of care performs accurate timely documentation in electronic medical record participates daily huddles Patient Care Coordination meetings prepares accurate timely reports as required for weekly meetings maintains continued competence nursing practice knowledge current evidence based practices may perform clinical tasks within scope practice performs all other related duties assigned
