Hospice Chaplain (4 CPE Units Required)

About the Job:We are currently seeking qualified and professional Chaplains to join our Home Healthcare agency. Our Home Healthcare agency services areas across all counties of the Bay Area. We have a current need for Chaplains who are eager and able to work in the Bay Area. We provide a comprehensive care program to each of our patients. To our staff, we offer flexible schedules, A friendly and professional environment, and a true team approach to healthcare.

Requirements include (but not limited to):

• Current CPR certification.
• 4 Units of Clinical Pastoral Education.
• Develop a plan of care for each assigned patient.
• Assesses patient needs, plans for, evaluates, and modifies care to meet goals of spiritual support.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.

HOSPICE CHAPLAIN Job Description:
