    Job Summary:
    Desire Home Care, Inc. is seeking a caring and organized individual to join our team as a Care Coordinator in Riverside, California.

    This is a full-time, hourly customer service position where you will be responsible for providing support and assistance to our clients and their families.

    As a Care Coordinator, you will play a vital role in ensuring that our clients receive the highest quality care and support from our agency.



    Compensation & Benefits:
    This is a full-time, hourly position with a competitive compensation package of $18 to $22 per hour, paid biweekly.

    Responsibilities:

    • Serve as the main point of contact for our clients and their families, providing exceptional customer service.
    • Coordinate and schedule care services for clients, ensuring that all needs and preferences are met.
    • Conduct client assessments and develop individualized care plans in collaboration with clients, families, and caregivers.
    • Partner with caregivers to ensure they have the necessary resources and support to provide quality care.
    • Maintain accurate client records and ensure that all necessary documentation is completed in a timely manner.
    • Serve as a liaison between clients, families, and other healthcare professionals to coordinate and manage care services.
    • Monitor client satisfaction and address any concerns or issues that may arise.
    • Participate in oncall rotation and provide afterhours support as needed.

    Requirements:

    • High school diploma or equivalent; Bachelor's degree in a related field preferred.
    • Minimum of 2 years of experience in customer service or healthcare coordination.
    • Strong organizational and timemanagement skills, able to multitask and prioritize tasks effectively.
    • Excellent communication and interpersonal skills; ability to build rapport with clients and caregivers.
    • Proficient in Microsoft Office and experience working with electronic health records.
    • Must be able to work fulltime hours and participate in oncall rotation.
