Community Developmental Services is a Division of Washington County Mental Health Services. We provide comprehensive services to children and adults with intellectual / developmental disabilities, autism, and/or other pervasive developmental disorders.
SUMMARY OF POSITION:
Provide Service Planning & Coordination to individuals with intellectual disabilities so they may gain access to services and supports in order to achieve successful outcomes. Examples of service provision includes, but is not limited to, developing, and implementing a person-centered plan, coordinating medical and clinical services, establishing and maintaining a case record, coordinating staffing supports for community and employment services, overseeing housing/home supports, and coordinating crisis services as needed.
