    Physical Therapist

    About the Role:

    The Physical Therapist at JAG Physical Therapy is a forward-thinking professional who excels at assessing patients' strengths and weaknesses to devise creative and effective treatment plans.

    Responsibilities:

    • Collects and records patient care data in accordance with department policies, regulatory agencies, and third-party payers.
    • Prepares for evaluations by obtaining patient history, researching unfamiliar diagnoses, and selecting and administering tests and measures.
    • Formulates diagnoses, goals, prognosis, and plans of care based on evaluation results.
    • Establishes treatment plans, collaborates with interdisciplinary teams, and assesses the need for modifications to plans of care and goals.

    Requirements:

    • Bachelor's or Master's Degree in Physical Therapy required; Doctorate degree preferred.
    • Licensed Physical Therapist in the state practicing in.
    • Completed state-regulated CEU requirements in appropriate states (NY, NJ).
    • Valid CPR License.
    • Experience/knowledge with EMR software preferred.
