    Description

    We are seeking a skilled nursing professional to join our team at Praha Village, a 91-unit senior living community in the South Metro. As an On-Call Licensed Practical Nurse (LPN), you will play a vital role in helping our residents experience independence and dignity within a loving community atmosphere.

    Job Summary

    In this rewarding role, you will provide compassionate care to our residents, administering medications and treatments as prescribed by physicians. You will also manage medications, supplies, and equipment, ensuring that all items are stored and disposed of in accordance with policies and procedures.

    The ideal candidate will have a current MN LPN license and prior experience working in an Assisted Living setting. Strong communication and organizational skills are essential for success in this role.

    Responsibilities

    • Administer medications and/or treatments in accordance with physician's orders and monitors for effectiveness/response
    • Manage medications, supplies, and equipment including ordering, receiving, storing, and disposing of all items in accordance with policies and procedures
    • Document care using computerized medication administration and charting software
    • Notify Clinical Manager, attending physicians, and family members of significant changes in a resident's condition, and takes follow up action as necessary

    Requirements

    • Current MN LPN License required
    • Prior experience working in an Assisted Living setting preferred
    • Eldermark experience a plus
    • Must have strong communication and organizational skills
