We are a large practice with over 25 providers that serve Skilled Nursing Facilities, Assisted Living Facilities and our homebound patients in the comfort of their home in 14 counties.

Our medical practice is currently looking for an outstanding Certified Medical Worker to join our Care Coordinator Team.

The Care Coordinator will work with our homebound patients and is responsible for meeting the patients and providers' daily needs to ensure an assigned home/color team patient census receives exceptional care.

Compassionate communication with provider, color team, patients and caregivers, coupled with strong documentation of all activities, forms the core of this position.

The Home Care Coordinator will be working closely with the Care Coordinator Manager and Lead in the office setting.


Skills:
