- Assesses physical condition of patient and evaluate patient for graded exercise.
- Contacts patients in a timely manner after receiving referrals.
- Educates patient on what services are available to them.
- Documents heart/lung sounds, musculoskeletal problems or concerns and status of incision site.
- Reviews medical history to assess problems with beginning physical activity.
- Designs and implements individualized exercise or activity plans for each assigned patient.
- Identifies learning needs of each patient.
- Reports progress on the patient care plan.
- Instructs the patient and family on appropriate home exercises/activities.
- Monitors progress during exercise sessions.
- Analyzes and interprets clinical signs and symptoms along with exercise EKG strips during the sessions.
- Adjusts the exercise plan accordingly
- Follows correct procedures in an emergency situation.
- Patient Education.
- Assesses patient's family's knowledge concerning cardiovascular risk factors.
- Develops and implements, in collaboration with clients and individual, realistic education plan.
- Conducts classes on heart disease and risk factor modification.
- Develops and utilizes cardiac rehab educational materials.
- Consults/collaborates with physicians and other health professionals regarding patient care, cardiac rehab education and program development.
- Communicates with patient's physician and/or other health care professional as needs dictate.
- Participates in meetings with medical director for patient care and quality improvement initiatives.
- Maintains department statistics and reports.
- Develops ongoing programs to deliver cardiac rehabilitation services.
- Maintain equipment.
- May set up equipment for each procedure.
- May train and provide in-servicing on all outreach accounts on equipment used, including downloading techniques.
- May trouble shoot outreach accounts equipment or connectivity issues.
- Other duties as assigned.
- Bachelor's degree in Exercise Physiology, Exercise Science/Kinesiology, Nursing or Occupational Therapy
- Master's degree in Exercise Physiology, Nursing or Occupational Therapy
- 1 year in cardiac rehab
- Licensed Registered Nurse - MN Board of Nursing required if registered nurse upon hire
- BLS Tier 1 - Basic Life Support - Multisource required within 60 Days
- ACLS-BLS Tier 2 - Multisource required within 60 Days
- Certified Exercise Physiologist preferred
