Job Summary Responsible for planning and performing high quality Occupational Therapy for the patients referred to Witham Memorial Hospital Occupational Therapy Department and its contractual clients.# Spends 80% of time in direct and related patient care activities and 20% of time in department related activities.# # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # Minimum Education/License/Certification Requirements Bachelors, Masters or Doctorate degree from an accredited Occupational Therapy Program.

Current certification or certification eligible by the National Board of Certification of Occupational Therapy (NBCOT).# Current license and in good standing by the State of Indiana or temporary license for new graduates.

Upon employment and yearly thereafter, must complete mandatory inservices.# Must be knowledgeable of disaster and safety procedures.# Will review the Rehab Services policy and procedure manual every two (2) years.# Shall be CPR certified in Basic Life Support, Course #C#.

Previous SNF or acute inpatient experience preferred.# Primary responsibilities will be in the skilled nursing facility located on the main hospital campus.# May float to inpatient setting during low census.
