    Description


    Overview Founded by the Sisters of Mercy, Mercy Hospitals have a history of caring for our community that goes back more than 100 years.

    We provide a broad range of medical and surgical services including minimally invasive and outpatient options.

    Our family of services includes the areas only inpatient oncology unit the Orthopedic Spine and Hand Center Family Birth Center the Lactation Support Center and retail store and many others.

    Mercy Hospital Downtown is a 194-bed facility located in downtown Bakersfield.

    Mercy Hospital Southwest is a 78-bed facility and the only hospital located in the rapidly growing area west of Highway 99.

    Mercy Hospital is a member of Dignity Health and is a trusted community partner serving residents of Bakersfield and Kern County with quality compassionate care since 1910.


    Learn more at Responsibilities Position Summary:

    Under the guidance of the Department Director, is responsible for oversight of daily operations to ensure the efficient and compliant departmental execution of the companies case management functions.


    These duties include, but are not limited to:

    staff performance management, reviewing or gathering of data to promote performance improvement, participation in or facilitation of meetings or projects related to departmental functions.

    Manages areas including organizing and executing all activities, staffing, performance improvement in the delivery of clinical services (such as LOS reduction), and reports needs within the Medical Center and Dignity Health, as well as government and regulatory reporting.

    Implements Care Coordination activities according to the needs, requirements, and policies of the Medical Center, the affiliated medical groups and health plans, Dignity Health, any Federal and State agencies, and according to standard practices of the professions under the manager's accountability.

    Consults and collaborates with other managers, physicians, administration, and community based healthcare workers regarding care management issues identified through corporate or facility initiatives and current literature.

    Oversees the department in an efficient manner to ensure timely and compliant care coordination, discharge planning, utilization review and social work interventions resulting in quality patient care.


    Qualifications Minimum Requirements:

    Current State License for Social Work (LCSW.) Masters Degree in Social Work from a university or college accredited by the Council of Social Work Education.

    Minimum of three (3) years in a hospital based Clinical Social Work role.
