    Visiting Nurse- RN/LPN

    Secure Home Health Care

    Hyde Park, MA

    Licensed Practical Nurse (LPN):

    • $32 to $34 per visit
    • Rates are based on experience and skills

    Registered Nurse (RN):

    • $36 to $40 per visit
    • Start of Care/Resumption of Care and Recertifications are paid at a higher rate
    • Rates are based on experience and skills

    Part-Time and Per Diem Opportunities (Opportunities for Full-time down the line as well)

    Job Description

    Secure Home Health Care is continuously seeking and hiring per-diem nurses (LPN/RN) to conduct home visits on behalf of our Home Health Agency.

    • Care coordination is paramount for us, and we ensure our patients get the best care and all clinical teams are on the same page no matter the case

    Job Responsibilities:

    • Assess patients' condition during every visit and chart your observations
    • Perform evaluation tasks, including vital signs and medication review
    • Administer medication as prescribed by the patient's Physician
    • Dress or redress open wounds and assess progress of healing
    • Educate patients and their families on proper home health care strategies and procedures
    • Coordinate with other care providers involved in the patient's care plan
    • Listen to the patient and respond to concerns or request

    Basic Requirements:

    • State RN/LPN license (Required)
    • 1+ years' experience in in-home or palliative care (Required)
    • Ability to travel to multiple patients per day (Required)
    • Excellent communication and time-management skills
    • Compassionate and friendly demeanor
