    Evergreen Village at Fort Wayne is seeking an Assistant Director of Nursing (ADON) to join our teamResponsibilities:
    Under the supervision of the Director ofNursing, our Assistant Director of Nursing (ADON) is responsible for training, scheduling, and guiding theclinical team toward delivering exceptional care experiencesConduct initial assessment of prospectiveresidentsPerform wellness assessments and developindividualized care plans based on the needs and preferences of eachresidentDeliver health promotion services, offer diseaseprevention counseling, and provide self-care education to residentsEngage with residents and family members in acompassionate and impactful manner, creating a lasting and positive experienceFoster communication by regularly engaging withphysicians, pertinent healthcare providers, and family members, ensuring theexchange of information on resident health statusContribute toward quality assurance andperformance improvement initiativesEngage in continuous learning and developmentopportunities through attendance and participation in educational seminars,staff meetings, and other learning experiencesComply with all rules and regulations regardingconfidentiality and privacy of team members and resident informationPerform any other duties as assigned ornecessaryQualifications:Holds valid state licensure as a LicensedPractical Nurse (LPN)

    Minimum of one (1) year supervisory experienceExceptional written and verbal communicationskillsPrior senior/supportive living experiencestrongly preferredDemonstrates exceptional interpersonal skillsand ability to serve withBenefits & Beyond:
