- High school graduate or equivalent (GED).
- Preferably one year's experience in healthcare or Hospice.
- Knowledge of Hospice philosophy and end of life care.
- Ability to provide knowledge and skills on the computer.
- Knowledge of Hospice policies and procedures.
- Ability to exercise independent judgment.
- Ability to work with individuals.
- Verifies that all required patient information is present prior to starting care.
- Utilizes appropriate clinical forms for entering data related to visit scheduling.
- Knowledge of hospice billing procedures.
- Submits timely computer documentation of all patient clinical visits.
- Responsible for answering telephone and forwarding calls to appropriate personnel.
- Promptly follows up with ordering equipment, communicating new Admissions to the Pharmacy, and Coordinating IDG members with regards to patient care concerns.
- Submits required documentation for billing purposes in the established time frame.
- Assists in implementation of policies and procedures.
- Observes confidentiality and safeguards all patient related information.
- Accepts responsibility for regular attendance and punctuality.
- File documentation/paperwork in appropriate chart
- All other duties as assigned.

Service Coordinator - Bingham Farms
Pay: $20 - $24 per hour
Schedule: Mon - Fri 8:30am - 5pm
JOB SUMMARY:
The Service Coordinator reports to the Hospice Administrator and possesses knowledge, training and experience in the Hospice Intake, Scheduling, and Coordination of Care Process.
