- Graduation from an accredited Physical Therapy program
- Two (2) years experience as a Physical Therapist preferred
- Recent / previous work experience in outpatient and/or inpatient experience preferred
- Ability to follow written and oral directions
- Computer skills required
- Demonstrates high level of personal and professional accountability and responsibility
- Self-directed and performs duties independently
- Ability to problem solve and apply critical thinking skills
- Must have the proven ability to maintain confidentiality
- Must demonstrate excellent customer service and communication skills
- Ability to effectively communicate and coordinate daily work flow to ensure department needs are met
- Must be flexible, dependable, and demonstrate the ability to adapt to change
- Current California or Nevada PT licenses and ability to obtain other state (CA or NV) license within 3 months of date of hire.
- Physical Therapist License Applicants may be considered, but full license for State of California must be obtained within 3 months of employment and State of Nevada license must be obtained within 6 months of employment.
- Current CPR/BLS certification by the American Heart Association (BLS for the Healthcare Provider) or ability to obtain prior to start date.
- While performing the duties of this job, the employee is frequently required to walk, stand, sit, and talk or hear.
- The employee is occasionally required to use hands to finger, handle, feel or operate objects, tools, or controls; and reach with hands and arms.
- The employee is occasionally required to climb or balance; stoop, kneel, crouch, or crawl.
- Specific vision abilities required by this job include close vision, color vision, and the ability to adjust focus.
- Lifts, positions, pushes and/or transfer patients.
- The employee must occasionally lift and/or move up to 50 pounds.

- Provides consistently exceptional care at all times.
- Performs initial and ongoing physical therapy evaluations according to APTA standards of practice as well as state standards.
- Develops a Plan of Care for each patient based on initial and ongoing evaluations including short and long term goals, collaboration demands of other staff members, and required patient and caregiver involvement.
- Implements Plan of Care on a consistent schedule and adjust this schedule and plan according to patient progress.
- Documents patient progress according to approved medical documentation and facility documentation procedures.
- Evaluates mobility, strength and current level of patient function, while setting goals to improve in these areas that are scaled to patient abilities and realistically attainable.
- Reports patient progress to patient, appropriate caregivers, facility staff and healthcare professionals involved in ongoing treatment/care of patients.
- Develops a plan for patients following completion of their Plan of Care with appropriate referrals.
- Assists other facility staff members as needed to provide care and create a clean, orderly and effective environment for rehabilitation.
- Assures continuation of therapeutic plan following discharge by designing home exercise programs; instructing patients, families, and caregivers in home exercise programs; recommending and/or providing assistive equipment.
- Documents patient care services by charting in patient and department records.
- Maintains patient confidence and protects hospital operations by keeping information confidential.
- Maintains a safe and clean working environment by complying with procedures, rules, and regulations.
- Protects patients and employees by adhering to infection-control policies and protocols.
- Ensures operation of equipment by completing preventive maintenance requirements, following manufacturer's instructions.
- Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies.
- Develops physical therapy staff by providing information; developing and conducting in-service training programs.
- Complies with federal, state, and local legal and professional requirements by studying existing and new legislation; anticipating future legislation; enforcing adherence to requirements; advising management on needed actions.
- Contributes to team effort by accomplishing related results as needed.
- Occasional travel or rotation assignment to various health system locations.
- Responds to the needs of the department by performing other duties, as necessary.

Summary of Position:
A Physical Therapist provides physical therapy services to patients utilizing established policies and procedures, ensuring safe and effective care. Services are provided based on the ages of the population served; Children through geriatric - 65 years or older and in collaboration with the multidisciplinary care team. Assesses, plans, implements and evaluates care in collaboration with the multidisciplinary team. Demonstrates the knowledge and skill necessary to identify and meet the individualized needs of all patients served. The Physical therapist is under the supervision of the Director of Rehabilitation and Sports Medicine and Rehabilitation Service Manager.
