    Summary

    Advanced Care Partners, one of Inc. 5000's fastest-growing companies, is seeking a compassionate and dedicated Direct Care Personnel to join our team at Advanced Care Pediatrics, a medical model childcare setting for children with special needs. You will play a vital role in providing direct care to our individuals, promoting their development and well-being through activities, support, and positive interactions.

    Responsibilities

    Supervises the provision of clinical nursing care to patients and provides care on an as needed basis.
    Manages clinical field staff
    Works with clinical staff to assist as needed with difficult care issues and to alter patient plans of care.
    Assess patient situations, staff problems, and coordinate physician referrals.
    Oversees proper maintenance of medical/clinical records.
    Oversees planning and evaluation of the Infection Control and Safety program for assigned patients.
    Assists Manager of Clinical Services with completion of all clinical paperwork related to patient care, including GAPP renewals.
    Conduct monthly supervisory visits and documents appropriately, according and compliant with state requirements
    Able to be on-call after hours, on weekends
    Identifies staff development needs and continuing education
    Assists in conducting employee performance evaluations/appraisals
    Assures accurate and timely completion of field employee incident reports, and performs audits of nursing notes
    Assists in the investigation of patient and customer concerns and complaints
    Maintains current knowledge of home care principles and practices, applicable laws, regulations, and practices
    Attends external meetings as needed and presents information on Advanced Care Partners Services to outside sources
    Maintains confidentiality of patient and agency information according to HIPAA guidelines.
    Qualifications
    Current Georgia Registered Nurse Licensure or Compact Registered Nurse Licensure
    Active CPR
