    Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Duchess, Ulster, Sullivan, Columbia and Westchester County.

    Sun River Health is currently seeking a full-time RAP Adherence Specialist in the Bronx, NY.

    Provides medical case management, adherence education, support, monitoring and follow up services to persons living with HIV patients.

    Will render care to patients as directed by the medical provider pursuant to the protocols of Sun River Health and grant standards .

    Utilizes motivational interviewing and a harm reduction approach in trauma informed care based interventions. Demonstrates cultural competency using an anti-racist framework and equity lens.


    Essential Duties and Responsibilities:

    • Facilitates patients who are no longer eligible for RAP and makes appropriate referrals
    • Reviews data reports to ensure accuracy and meeting grant deliverables
    • Conducts intake, comprehensive assessments/re-assessments and evaluations for Rap patients
    • Develops and implements retention in care strategies
    • Provides appropriate follow up on patient referrals, missed appointments etc.
    • Provides health education and risk reduction to patients within scope of discipline to assist in patient engagement and retention and reduce barriers to care to enhance health outcomes
    • Provides ongoing follow up and support to patients who are not virally suppressed
    • Maintains data and case records as required and prepares necessary reports
    • Develops and monitors care plans for RAP patients
    • Participates in ongoing training and regional collaborative s to enhance knowledge and skills

    Experience:

    Knowledge of nursing interventions, HIV signs and symptoms, assessment, and treatment options; competence in working with HIV+ patients, medication protocols, and triage.

    Demonstrated experience working with high risk groups.

    Bilingual in English and Spanish is preferred but not mandatory.

    Bachelor's Degree
