    Job Description
    • Creates and promotes a culture centered on building self-directed lives with the people we support.
    • Displays principles of good management and applies these in daily work performance
    • Serves as the liaison between Supreme Nursing Care and Supported Living and supports coordination agencies as well as county administrative entities
    • Coordinates monthly meetings and emergency team meetings with outside supports coordination agencies
    • Attends internal treatment team meetings and bi-weekly management meetings
    • Tracks data on ISP outcomes, behavioral outbursts, community participation and health promotions
    • Creates monthly and quarterly progress reports
    • Produces annual assessments based on the individual's progress and growth
    • Writes restrictive procedure plans for individuals with complex behavior concerns
    • Makes independent decisions regarding program operations with his/her authority
    • Notifies the Lead Program Specialist in an accurate and timely manner of unusual incidents, problems, and changes in individual Person Centered Plans
    • Promotes a positive working relationship with all employees
    • Coordinates and reviews the careful preparation with individual's records and ensures that they are kept accurate and up to date
    • Provides direct support to individuals as needed
    • Ensures that programmatic routines are followed as established; follows established procedures when making changes
    • Remains aware of behavioral and /or medical status of the people that we support
    • Records and reports to the Lead Program Specialist
    • Participates in required in-service training and professional growth activities
    • Provides counseling and assistance to individuals as needed
    • Completes all other assigned tasks by Person Directed Supports, Inc.
